These are specialized service conducted by a qualified pharmacist with a Home Medication Review Certification. They can attend your home after the Doctor has written a referral to request for any of these services.
This is done in 3 stages which involves; the initial interview, then 1st and 2nd follow up. From each visit a report is sent to your Health Professional to assist with Patient’s treatment Management.
In this package our pharmacist will be able to go through medication in a greater detail and explain to the patient about their prescriptions, over the counter medicines and their interactions. This service is covered by Medicare. Please do not hesitate to contact us if you have any questions before booking this service.
Mut Home Pharmacy introduced this service for our clients who want to get general knowledge about their medications and how they work and interact. Unlike Medication Reviews, this package is not covered by Medicare and you do not require a referral from the Doctor for this service.
This service is conducted by our Pharmacists and final year Pharmacy students who have accumulated knowledge and understanding in clinical areas pertaining clients / patients’ medicines. In this service you will be able to receive a report which can be forwarded to your Health Professional and also comes with a follow up package if required.
An experienced Pharmacist or Final year Pharmacy student will come to your home and talk to you about the medications you are taking including prescriptions and over the counter Medecines and their interactions. We have tailored our best packages with intentions of providing good services and developing good relationships with our patients and customers. After 2 two weeks, our qualified Pharmacists will endeavor to follow up with a phone call.
This is a test specially for our patients who take Warfarin and the Doctors might request for INR tests for different reasons but we have mobile devices that can give instant results and you don’t need to wait. It’s only a small little prick on the finger.
Similarly to how a Blood Glucose test is done.
Mut Home Pharmacy is equipped with devices and technologies to make the lives of our clients/ patients much easier. You will also have a chance to speak to our Pharmacists and final year Pharmacy students about your medication including vitamin K and Warfarin mechanism of action.
Mut Home Pharmacy is equipped with mobile portable Cholesterol machines. It’s a simple test similar to Blood Glucose machines. This service is conducted by our qualified Pharmacists and final year Pharmacy students.
This service can be a referral from the Doctor or it can be offered to patients and healthy individuals that are keen to monitor their cholesterol levels. Our qualified and final yr Pharmacy students will have the opportunity to speak to you and provide counseling in relation to cholesterol and medications.
In this service we are screeing for patients who are presenting with symptoms of UTIs. Through this service, a small sample of mid- stream urine is collected by our Pharmacists and final year Pharmacy students and tested, the results are released within 5 minutes.
Our staff will then explain the investigations observed in urine which might indicate symptoms of UTI or other kidney or bladder related infections. After this test you will recieve a report that you can present to the doctor. Please do not hesitate to give us a call for clarification.
These are standard Blood Glucose and Blood Pressure monitoring packages. With the Blood Glucose test, you may be referred by your Health Professional, However a referral is not required for these services. Upon booking please let us know whether it is a Fasting Blood Glucose/ Non-fasting Blood Glucose test.
For Blood pressure monitoring, our staff will explain these numbers to you and whether this needs to be followed up by a Health Professional. A follow up visit is available at a discount fee if further monitoring is required. For our patients on Blood Pressure medications in Age care facilities and Disability supported living homes who have carers or family members caring for them, we suggest you book in for our specialized training for Hypertension where our Qualified Pharmacist will go through Blood Pressure in greater detail.
